CiminoCare: Leading the State in Third-Party Payer Relationships

At CiminoCare, our mission has always been rooted in increasing access to high-quality assisted living and memory care — especially for older adults who rely on Medi-Cal or lack the private funds typically required for long-term residential care. We believe that every older adult deserves dignity, support, and community — and we’ve built strategic partnerships that help make that belief a reality.

CiminoCare was an early supporter of California’s Assisted Living Waiver (ALW) program, recognizing its potential to bridge the gap for Medi-Cal beneficiaries in need of residential care. Over the past five years, we have expanded our third-party payer relationships to create innovative pathways for care that didn’t exist before.

Today, CiminoCare partners with a range of providers and programs to remove financial barriers for frail older adults:

  • Letters of Agreement (LOA) with hospital systems to support transitions from acute care into assisted living and memory care
  • Contracts with PACE (Program of All-Inclusive Care for the Elderly) and other community-based enhanced care management programs
  • Collaborations with managed care organizations including San Mateo Health Plan, Health Plan of San Francisco through Institute on Aging programs, and Kaiser CalAIM Anthem, Partnership Health, and Health Net through MasterCare contracts.
  • Most recently, CiminoCare earned credentialing with Anthem Managed Care to provide three community support services — expanding both short-term and long-term assisted living and memory care options for older adults without private resources.
These partnerships reflect not just innovation, but urgency: California is facing a growing need for housing with care options for low-income older adults. According to recent statewide data, the number of Californians age 65+ is projected to grow rapidly over the next decade, and many will require supportive residential care. Yet a significant percentage of this population lacks the financial means to access it. Medi-Cal remains the primary insurer for low-income older adults, but traditional assisted living and memory care often fall outside covered benefits without programs like ALW, CalAIM, and strategic managed care partnerships.

Mark Cimino, CEO, has been nominated for three Department of Health Care Services (DHCS) committees to look at how to expand services to frail MediCal recipients. “We expect to see a lot of changes in the various MediCal-funded programs over the next few years.  CiminoCare is at the table, participating in several stakeholder groups for these conversations, advocating for access to this level of care,” states Mark Cimino, CEO.

By bridging the gap between public programs and community-based care, CiminoCare is helping to reimagine how assisted living and memory care are funded, delivered, and experienced — making compassionate care more equitable and accessible across the state.

At the heart of this work is our commitment to serving each person with respect, connection, and choice — not just meeting care needs, but supporting a continuum of services that includes housing, personal care, medication management, end of life and enhanced care management.

Medi-Cal has long covered nursing home care, but historically offers limited coverage for assisted living and memory care without specialized programs like the Assisted Living Waiver (ALW) which are historically slow to respond to increased need.

The Solution

Third party payers including PACE and Managed Care partnerships are helping close this gap through the CalAIM Community Supports, creating new pathways to assisted living and memory care for older adults who cannot afford private pay options.
